Application Process

1. Please print and complete the forms below and leave with the school secretary.  
   • The secretary will have the forms signed by the building principal.
2. The student or parent will be notified when the forms are ready to be picked up in the office or they can be mailed to your home address if requested.
3. Take all documents to the Iowa DOT to obtain the school driving permit.
